Do you provide a service, activity or a group in Bridgnorth? Would you like to know more about Social Prescribing and how you can get involved? Bridgnorth Medical Practice is working with Shropshire Council to bring Social Prescribing to Bridgnorth and invites you to an event to find out more.
What is the reason for Social Prescribing?
Many people are in situations that have a detrimental effect on their health and often want to improve their situation, but need support and motivation in order to make a positive change.
It is estimated that around 20% of patients consult their GP for what is primarily a social problem.
Social prescribing facilitates the use of non-clinical support and provides a link to local community support. It provides General Practice with a structured referral pathway into local, non-clinical, community activities, services and support. The programme offers more than signposting, as it includes support from an advisor, along with data recording and quality assurance.
Resiiient Communities?
Social Prescribing in Shropshire is part of the Healthy Lives Programme and the Resilient Communities work stream. Shropshire Council’s Community Enablement Team are delivering Resilient Communities and have been working closely with Shropshire Councillors, fellow council officers, NHS colleagues and Voluntary Sector Organisations to design targeted interventions and activity in localities.
Resilient Communities is all about connecting up people and activities in our neighbourhoods so that we all know more about what is going on and have the choice and opportunity to get involved in the things we are interested in, making us happier and healthier. By doing this we aim to build on the existing resilience in Shropshire’s communities so that people are able to live independent, safe and fulfilling lives in their local community.
