We were asked: Is this something that a child would automatically move into at 16 or do you have to apply and fight for it ?
This is the reply we received from Preparing for Adulthood Lead Adult Social Care:
The team offers support to a range of young people under the age of 25 years, although young people with learning disabilities are the majority of young people receiving support from the preparing for adulthood team. We work closely with our colleagues in adult social care mental health team to support young people accessing their support and also support young people with physical disabilities and autism.
