NHS Shropshire Clinical Commissioning Group (CCG) will be in Shrewsbury Darwin Centre (ground floor) to meet local shoppers to find out what matters most to local people as the CCG seeks to develop a new healthcare service.
From 1 pm to 4 pm, on Monday 25th February, CCG staff will be talking to people and asking for their feedback to help shape and develop the new Shropshire Care Closer to Home Service.
This service aims to help local people over the age of 65, who have a number of health conditions and receive regular care and support from the NHS and Shropshire Council. It aims to help them get their care in the community, and even in the comfort of their own homes, which could also mean they may avoid a stay in hospital.
The Shropshire Care Closer to Home Team is developing this new service to join up health and social care. They will be on hand to talk about how the new service could possibly look and to get feedback as well as seek ideas to help develop it.
They especially want to know what matters to local people over 65 who have day-to-day experience of care and support from the local NHS and Shropshire Council and if there is anything that could be changed or done differently.
