Healthwatch Telford & Wrekin and Healthwatch Shropshire are working together to gather patient, family and carer experiences of those who have received ‘out of hours’ palliative care since the beginning of March 2020. We would like to gain an understanding of the needs and experiences of palliative care patients and their carers in order to help ensure that out-of-hours care is both effective and efficient.
Have you used the ShropDoc Palliative Care Helpline, NHS 111 or any other service and might be interested in sharing their experience to help improve services?
About the survey:
The first part of the survey asks for a few details about you to help us understand who is using the services.
The second part of our survey asks some questions about your experience of accessing palliative care support from health services during the time when your GP surgery is closed, over-night, weekends and bank holidays.
It should take approximately 5 to 10 minutes to complete depending on the detail of your responses.
All responses are anonymous.
If however you would like to discuss your experiences with the service providers their contact details are provided at the end of the questionnaire.
