The National Audit of Dementia assesses the quality of care provided to patients with dementia when they are in hospital. If you are visiting someone with dementia in Royal Shrewsbury Hospital or Princess Royal Hospital (or another participating hospital) between 4th June and 21st September, please complete a survey about your experiences.
If you cannot complete the survey on-line, at this link https://rcop.formic.com/WebForm.aspx?ID=8572AA647CCE4F4286D9B0341B21FAE6 please ask a member of staff at eith the Royal Shrewsbury Hospital or Princess Royal Hospital for a paper copy of the survey.
The survey is very important as the National Audit Team, will use the information to highlight areas of improvement that hospitals need to focus on.
