http://www.ceacard.co.uk This is a national card that can be used to verify that the holder is entitled to one free ticket for a person accompanying them to the cinema. Costs £5.50 and is valid for 1 year, so have to renew each year. You must be getting DLA. Info and downloadable application form on website.
